LASIK Consent Form

Before Your LASIK

1. No contact lens wear prior to LASIK as follows:
a. No hard or Gas perm lenses 4 weeks prior to evaluation and LASIK.
b. No extended wear and toric soft lenses 1 week prior to evaluation and LASIK.
C. No daily wear soft lenses 3 days prior to evaluation and LASIK.

2. Make-up schedule as follows:
a. Avoid all eye make-up 3 days prior to LASIK.
b. Avoid all make-up on day of LASIK.
c. Avoid mascara and eyeliner 1 week after LASIK.

3. Wear loose and comfortable clothing.

4. Avoid perfume, after-shave and heavily scented deodorant or body lotion on the day of LASIK.

5. Eat a light meal on the day of LASIK and avoid alcohol.

6. Be prepared to spend about an hour at the LASIK Center .

7. Arrange transportation with family, a friend, or the front desk.

8. Avoid eye ointments for one day before LASIK.

9. Take all regular medications unless otherwise directed.
